Ultrasound-Guided Procedures
Ultrasound-guided diagnostic and therapeutic procedures enable doctors to locate, examine, and treat soft tissue damage and injuries with more accuracy and precision. US- guided injections help in placing the needle correctly to maximize outcomes in the damaged area and minimize patient discomfort. Dr. Paul performs several ultrasound-guided procedures, including:

Bone Marrow Aspirate Concentrate (BMAC) Injections
BMAC injections are used to reduce pain and inflammation, restore functional mobility and promote natural healing for several orthopedic conditions, including osteoarthritis, joint pain, cartilage injuries, and tendinopathies.
Bone marrow is a rich source of adult stem cells, both mesenchymal (MSC’s) and hematopoietic (HSC’s), which have the ability to replicate and develop into different types of cells as well as influence the microenvironment of the tissue or joint via the paracrine effect. Stem cells contain growth factors and anti-inflammatory proteins that promote natural healing, repair and support in areas of damaged or injured tissue. Micronized Lipoaspirate Injections

Adipose tissue is the body’s fat tissue. It’s a valuable source for cell-based therapies because it’s abundant, easily accessible and harvestable, and it’s rich in mesenchymal stem cells (MSCs), vascular cells, and collagen fibers, when mechanically fragmented (non-enzymatic). Like other types of adult stem cells, MSCs have the ability to replicate and develop into different types of cells. During the procedure, Dr. Paul removes a small portion of fat using image-guided liposuction. The adipose tissue is processed mechanically, micronized precisely and non-enzymatically, to remove inflammatory cells and fatty oils, creating microfat, and leaving only the MSCs and other helpful cells. Alpha 2 Macroglobulin (A2M) Injections

Alpha 2 macroglobulin is a large plasma protein found in blood. It functions as a protease inhibitor, which means it blocks the activity of degradative, proinflammatory proteins that cause progressive degeneration in arthritic joints, cartilage and bone. A highly concentrated injection of A2M can prevent the progression of damage and cartilage breakdown, promote new tissue growth, and encourage natural repair and restoration of the degenerated joint.

Nerve hydrodissection
- Nerve hydrodissection is a treatment technique commonly used in pain management and regenerative medicine. It is an ultrasound guided procedure that uses fluid injection under pressure to separate the nerve from the surrounding tissues to reduce nerve entrapment and irritation and neural inflammation. This can result in the relief of chronic pain and other symptoms caused by nerve entrapment.
The procedure is usually performed using ultrasound guidance to ensure accuracy and safety. The fluid used can be a local anesthetic, saline, or sometimes substances like dextrose or Platelet-Rich Plasma (PRP), Platelet Poor Plasma (PPP), Alpha-2-Macroglobulin (A2M), or saline depending on the specific situation and desired effect. These fluids are injected around the nerve, creating a small space that allows the nerve to move more freely, reducing friction and irritation.

TENEX/TENJET Percutaneous Needle Tenotomy
- A minimally invasive sutureless percutaneous (no incision) medical procedure used to treat chronic tendon pain.
- It involves using ultrasound or high pressure saline stream technology to break up and remove damaged tissue from the tendon with saline lavage and aspiration of the damaged tissue.
- The procedure is typically performed under local anesthesia and takes about 15 minutes.
